Updating your information
It’s important to update your personal information whenever your name, address, or Social Security Number changes. Here’s how:
Medical and other coverage
Email Ask HR (AskHR@Levi.com) or call +1-844-474-5384.
Savings plan and pension benefit
You must call Fidelity at +1-800-354-6560 to update personal information (name, SSN, phone number) for your savings plan and pension benefit.
Address and email changes can be made online with Fidelity at NetBenefits.com.
If you need a new ID card, just give your medical plan provider a call:
Keep your beneficiaries current so your insurance goes to your intended loved ones.
Here’s how:
Call Ask HR at +1-844-474-5384 anytime to report life-changing events like divorce, a dependent losing coverage, or a death.
Keep in mind: Some life changes might affect your retirement benefits, so report any life change or death of a family member by calling Fidelity (+1-800-354-6560).
Making changes to your coverage
Your coverage needs may shift as life changes. You can make changes to your coverage for yourself or your dependents anytime, for any reason. Contact Ask HR to waive or terminate coverage, or switch to a lower-cost plan.
Terminate coverage
You can end coverage for yourself and your dependents anytime, for any reason. If you terminate your coverage, you will not be allowed to reinstate your coverage at any time. Your coverage will be terminated the first of the following month in which Ask HR receives the termination form. Once you terminate coverage for yourself, coverage for your enrolled dependents will also be terminated.
Waive coverage
You can waive retiree coverage, with the opportunity to elect coverage at a later time. Your coverage will be waived the first of the following month in which Ask HR receives the completed waive form. Once you waive coverage for yourself, coverage for your enrolled dependents will also be waived.
You must waive your LS&Co. health benefits if you take another job with group health benefits. Contact Ask HR as soon as possible. To re-enroll in LS&Co. retiree health benefits after your other coverage ends, notify Ask HR within 31 days of when your other coverage ends.
Switch to a lower-cost plan
You can switch to a medical plan with a lower monthly premium if your needs change, as a one time change. Your dependents’ coverage will change with yours, and your pension deductions will be adjusted accordingly.
Yes, you can terminate your coverage anytime for any reason. Just know you won’t be able to re-enroll in LS&Co. supplemental life coverage later. If you terminate your supplemental life, you will not be allowed to reinstate your supplemental life at any time. Your coverage will be terminated the first of the following month in which Ask HR receives the termination form.
There are two ways to pay for your coverage:
Pension deduction:
Your contribution can be deducted from your monthly pension check. You’ll get a letter about the amount and due date for your premiums before your first pension check arrives. Note: Your first premium can’t be deducted from your pension, so you’ll receive a one-time bill from Inspira Financial.

Note: In certain situations, Inspira Financial will direct bill. If you defer your pension or your pension does not fully cover your contribution, you will receive a monthly bill from Inspira Financial. The mailing address and contact information is listed above.
Managing your pension and savings
You can set up or change your direct deposit information online with Fidelity.
Call Fidelity at +1-800-354-6560 to request a replacement check or to report forgery.
Call Fidelity at +1-800-354-6560 to obtain verification of your pension.
Beneficiaries must call Fidelity at +1-800-354-6560 to update personal information (name, SSN, phone number) for your savings plan and pension benefit.
Address and email changes for your pension also require a phone call. A beneficiary can update their address or email for the savings plan online if they have an account.

Tax forms and withholdings
If you opted for e-delivery with Fidelity, you can receive your 1099-R by logging into your account on Fidelity’s website. If not, you must call Fidelity at +1-800-354-6560 to ask for your 1099-R.
Your 1095-C will be mailed to the address we have on file for you. If it doesn’t arrive, contact Ask HR at +1-844-474-5384.
You can change the amount of your tax withholding by calling Fidelity at +1-800-354-6560.
Turning age 65 and medicare
Once you turn 65, you’ll need to enroll in Medicare, or you’ll have no medical coverage. Your LS&Co. retiree medical coverage ends on the last day of the month before your 65th birthday. You are eligible to enroll in Medicare six months prior to and six months following your 65th birthday. It’s best to apply early to ensure coverage.
Yes. To ensure you have continuous prescription coverage, LS&Co. automatically enrolls UHC medical plan participants in prescription coverage through Optum Rx when you (or eligible dependents) turn 65. This gives you time to explore your options and decide what works best for you. You can waive or terminate this automatic prescription coverage at any time.
You automatically receive a Medicare application when applying for Social Security benefits. If you decide to defer your Social Security benefits, initiate your Medicare enrollment with Social Security by phone or online.
