Medical benefits for retirees under age 65

Medical

Your retiree medical plan is based on the same plan and level of coverage that you last elected as an active employee. Forgotten which medical plan you have? You can find your medical plan name in the letter from LS&Co. that you recently received in the mail. The retiree medical plans are:

Your pre-65 retiree medical coverage will end a month before your 65th birthday. At that time, you’ll be eligible for prescription-only benefits for retirees age 65 and older.

UnitedHealthcare ID card

You must use your retiree ID card for all medical claims. If you need care before you receive your new ID card, give your provider UnitedHealthcare’s retiree medical plan ID number: 923812

Prescription drugs

For retirees enrolled in an LS&Co. medical plan, prescription drug coverage is provided through OptumRx.

OptumRx ID card

You must use your retiree ID card for all prescription claims, or your prescriptions won’t be covered.

Call OptumRx at +1-855-475-5082 if you need a new ID card. And if you need care before you receive your new ID card, give your provider the OptumRx retiree medical plan ID number: 923812.

Integrity Pharmaceutical Advisors (IPA)

Dental

Unlike your retiree medical coverage, your dental coverage (through Aetna) will not be canceled when you turn 65 years old, as long as you keep your premiums current.

You can suspend or terminate your dental coverage anytime. Important: If you terminate your coverage, you will not be allowed to reinstate your coverage at any time.

Vision

All retirees are eligible for the EyeMed Vision discount program, which includes discount prices on eye examinations, glasses, lenses and more when you go to a participating provider. Plus, you don’t need to enroll to participate.

Cost for coverage

Medical and prescription drugs

Both you and LS&Co. share in the cost of your retiree medical coverage. The amount you pay for coverage depends on:

  • Your years of service
  • The overall cost of the plan you’re enrolled in (you pay 100% of any cost that exceeds the annual per-person maximum that LS&Co. contributes)

Your premium is based on a per-person rate. This means you pay one amount for your coverage and one amount for your spouse’s coverage. If you cover any eligible children, you’ll pay an additional cost of $100 per month, whether you cover one or more children.

Dental

The cost for dental coverage is covered under the retiree welfare plan.

Change in coverage

Suspend coverage

  • You must suspend your LS&Co. health benefits if you go to work for another employer and are eligible for group health benefits through them. Coverage for your dependents will also be suspended.
  • To suspend your LS&Co. health benefits due to coverage through another employer, sign the Notice to Suspend Coverage form, and return it to Ask HR as soon as possible. There will be no deductions to your pension check while your coverage is suspended.
  • If you suspend your coverage and then want to reenroll in the LS&Co. plan, notify Ask HR within 31 days of the date that your other coverage stops.

Terminate coverage

  • You can terminate your or your dependents coverage at any time for any reason. Coverage for your dependents will also be terminated, and you cannot ever reenroll in the LS&Co. retiree health plan. If you need to terminate your dependents coverage only, contact Ask HR
  • To terminate your LS&Co. health plan coverage, sign the enclosed Notice to Terminate Coverage form, and return it to Ask HR within 31 days of the date of your retirement.
